Not sure if you've already been here, but this website has some info under "I am a qualified patient. How and what documentation do I need to apply for a MMIC?" Basically, it says that you must apply in person at your county's program. https://www.cdph.ca.gov/Programs/CHSI/Pages/MMP-FAQS.aspx
